Welcome to the Continuous Care Surveillance Tracking tool
The Continuous Care Surveillance Tracking (CCST) tool is designed to collect data by creating events related to Surveillance activity. DOH embedded staff in the Nursing Home, Adult Care Facility, Immediate Care Facility for Individuals with Intellectual Disabilities and the Psychiatric Residential Treatment Facility programs are required to enter their survey activities on a weekly basis. Surveillance staff must ensure the accuracy of all data prior to the closure of an event.
If there is any difficulty creating/completing an event, contact the help desk for assistance. Do not create a new event unless directed to do so.
Requests to edit events, after closure, must be made through Dakota Kimble and/or the help desk for review and assistance.
